1) Car Certificate of Insurance (CI)
2) PARF Rebate / Vehicle Log card
3) Owner + Driver's IC (Physical or Singpass)
4) Owner + Driver's Driving License (Physical or Singpass)
5) Completed Accident Reporting form standardised by GIA
(To be filled up during accident reporting at your reporting centre)
6) Medical Certificates, Receipts & Invoices and Doctor Letter/Diagnosis (if any injury from incident)
